Overview
We serve both the patients of Grace Health and our community. Our staff strives to make the pharmacy experience comprehensive, convenient and informative. We will work directly with your provider to make sure we are well informed about your condition and treatments. We focus on patient education, making sure you understand all of the medications and the conditions they are meant to treat. Our goal is that every patient who comes to our pharmacies receives the safest, most cost effective medication.
Patients of Grace Health who financially qualify for our sliding fee program will also be able to receive a sliding fee discount in the pharmacy as well. Please ask the pharmacy staff about our sliding fee program.
Grace Health pharmacists are specially trained to take a more active role in our patients’ care. We work directly with patients who have chronic health conditions, like diabetes, to evaluate drug therapy and promote better outcomes. We also support other Grace Health providers by participating in Annual Wellness Visits that encourage preventative health.
The Pharmacy Clinical Care Center is a team dedicated to patients access to their medications. By focusing on refill requests and other medication related issues, the team helps patients better understand and get the most benefit out of their medications.
